Office of Workers' Compensation (OWCP) Forms

Horizon Multispecialty Clinic, has provided a variety of forms available online. We are listing the most common forms below but an extensive list can be found at the Office of Workers' Compensation Programs website.

Frequently Asked Questions

If you had employment-related injuries or occupational diseases you might qualify for one of the four major disability compensation programs the Office of Workers' Compensation Programs (OWCP) offers. Our providers will help and treat Federal Employees who have been hurt on the job. The providers will help the injured patient to recover quickly, guide, and assist with OWCP claim process. When you are injured due to a work-related injury, it is important to be treated by a provider who is familiar and experienced with the proper OWCP procedures and the DOL-OWCP system requirements. This will assist you in getting your claim faster and accurate, without delays or denials. Our Frequently Asked Section provides some common answers regards the form to be filled. Feel free to call us for further assistance.

  • How to file a claim CA-1 or CA-2?

If you wish to file a claim under the Federal Employees' Compensation Act (FECA), you can file a claim using Form CA-1 (Notice of Traumatic Injury and Claim for Continuation of Pay/Compensation) or Form CA-2 (Notice of Occupational Disease and Claim for Compensation). Depending on your agency, you may be required to file an OSHA-301 before filing a FECA claim.

If your claim is submitted to the Office of Workers' Compensation Programs (OWCP), you will be assigned a case number and receive an informational letter from OWCP with that case number. That letter will also provide you with information regarding how to view your case status, billing updates, coverage limitations, and other information via the Claimant Query System (CQS).

Once a case number has been assigned, you can submit information pertaining to that case file through ECOMP's Document Upload feature, available from the ECOMP homepage.

  • The CA-1 form is used if the employee has sustained a Traumatic Injury on the job.

To be eligible, you must submit a CA-1 within 30 days of the injury. If disabled and claiming OWCP benefits, you must submit medical evidence supporting your disability to your employing agency within 10 workdays.

The injury is a medical condition resulting from an incident or activity occurring during one work shift.

  • The CA-2 Notice of Occupational Disease form should be used if you have sustained an occupational disease injury on the job. An Occupational Disease is a condition produced in the work environment over a period longer than one work day or shift.

To be eligible, you must submit a CA-1 within 30 days of the injury. If disabled and claiming OWCP benefits, you must submit medical evidence supporting your disability to your employing agency within 10 workdays.

  • Form CA-1 is used for a traumatic injury (a medical condition resulting from an incident or activity occurring during one work shift). Form CA-2 is for an occupational disease (a medical condition resulting from an incident or activity occurring over more than one work shift).

  • • An ECOMP account (you can register for an ECOMP account using your email address)
    • Your government organization
    • Your supervisor's email address
    • For new injury/illness claims, details for the event (place, time, extent of injury, etc.)
    • If available, supporting documentation, such as medical reports (have these ready in electronic format)

  • Remember that before filing a form in ECOMP, you must register and create an account. Once you have registered, you can initiate a form from your Employee Dashboard.

You must fill out each form completely and submit it for review. You may save a partially completed form and come back to it later. You may also upload and attach supporting documents to be submitted along with a FECA claim form. If your form is incomplete, it will appear in your Dashboard as 'DRAFT', and will not be processed until you complete all fields and submit the form for review.

Once you submit your form, it will be reviewed by your supervisor (we'll ask you for their email address) and then by a reviewing official within your agency. If appropriate, FECA forms will then be submitted to OWCP. If your supervisor has a question, he/she may send the form back to you. Forms sent back for your review will appear in your Dashboard as 'Returned to Filer'.

You will receive status emails during this process. You may also use the "Track Status" function to monitor the progress of your submission or check the status of your forms from your Employee Dashboard.

Everyone is always so nice.

I’m a VA worker and after a 1 ½ year of frustration going to a clinic that promise me to help me with my claim unsuccessfully. I was recommended by my workers comp lawyer to go to Ms. Brenda at this multispecialty clinic and in 6 month my claim was open, and all my conditions were accepted. The provider fought for me and wet above beyond to help me with not only my claim, but with my medical conditions and the anxiety this was cussing to me. The staff is very nice and well vested in Federal Workers Comp paperwork. Thank you, Horizon clinic.

Barbara Nollie

Met my medical needs promptly. Greeted me respectfully when I came inside. Nice visit.

I am a DOL worker myself. After an on the job driving accident other than the care I received in the ER from the ambulance trip, I was never given any copies of any forms for follow-up care or treatment, including a traumatic brain injury. The accident happened on 10/26/2018. Thank goodness for health insurance. Yesterday, 5/28/22, I presented myself for treatment from excruciating pain in my lower back. I was ready to leave and in tears because I have spent years trying to get one care provider to pick up the phone and call my claims examiner, FINALLY, the team at Horizon did exactly what needed to get done. Even FOI no longer cares for OWCP patients. Florida Pain Relief Group also has zero clues how to fill out the federal forms and provide the detailed medical explanations of what OWCP even needs. God forbid they pick up the phone to call OWCP. Hopefully I can start getting the care and treatment I need and deserve. My emotions were so out of control and they saw through my tears to try to help me. All workers deserve this type of compassion when they are already down and out. Ms. Connie, Dr. Sullivan and the rest of the staff, thanks again!!! I may not be out doing the parade today, but the relief has begun!!! Thank you all.
